About this test
What is Ovulation / Fertility Monitoring Ultrasound?
- A non‑invasive imaging test that uses sound waves to create a picture of the ovaries and uterus.
- It checks the size and number of follicles, the thickness of the endometrium, and blood flow to the ovarian tissue.
Key Parameters
| Parameter | What it shows |
|---|---|
| Follicle count | Number of developing follicles (usually 1–12 per ovary) |
| Follicle size | Diameter of each follicle (mm) |
| Endometrial thickness | Thickness of the uterine lining (mm) |
| Blood flow (Doppler) | Blood supply to ovaries and uterus |
Why is it done?
- Routine fertility evaluation in couples trying to conceive.
- Assessment of ovulation timing and quality.
- Monitoring response to fertility drugs (clomiphene, letrozole, gonadotropins).
- Evaluation of ovarian reserve and potential ovarian cysts.
- Helps plan timing of intrauterine insemination (IUI) or IVF.
Early Detection
This test can help identify signs of ovarian cysts, polycystic ovary syndrome (PCOS), endometrial hyperplasia, and poor ovarian reserve, but it does not diagnose these conditions on its own. A doctor interprets the results together with symptoms, history, and other tests.
What an abnormal result may mean
- High follicle count or large follicles may suggest PCOS or a cyst.
- Low follicle count or small follicles can indicate diminished ovarian reserve.
- Thin endometrium may reduce implantation chances.
- Abnormal blood flow can point to vascular issues or ovarian dysfunction.
Many factors such as hydration, recent medication, or hormonal fluctuations can affect the findings.
Who might be advised to get this test?
- Women aged 20–40 trying to conceive.
- Women with irregular periods or suspected ovulatory disorders.
- Women undergoing fertility treatment.
- Women with a history of ovarian cysts or PCOS.
Summary: An ultrasound of the ovaries and uterus provides a visual snapshot of ovarian health and helps guide fertility planning.

Report parameter guide
Antral Follicle Count
Number of small follicles seen in the ovaries
Low: May indicate low ovarian reserve or diminished fertility
High: May indicate polycystic ovary syndrome (PCOS) or increased ovarian activity
Ovarian Volume
Size of each ovary
Low: May suggest low ovarian reserve or underdeveloped ovaries
High: May suggest enlarged ovaries due to cysts or PCOS
Endometrial Thickness
Thickness of the uterine lining
Low: May indicate insufficient uterine lining for implantation
High: May indicate thickened lining due to hormonal imbalance or early pregnancy
Follicle Size
Diameter of the dominant follicle
Low: May indicate an immature follicle not ready for ovulation
High: May indicate a mature follicle ready to release an egg
Corpus Luteum Presence
Whether a corpus luteum is seen after ovulation
Low: May indicate no ovulation has occurred
High: May indicate recent ovulation and luteal phase activity
