About this test
What is a Prostate Biopsy with Ultrasound Guidance?
A prostate biopsy is a medical procedure that collects small tissue samples from the prostate gland. Ultrasound guidance uses real‑time imaging to help the doctor locate the exact area to sample, improving accuracy.
What it checks
| Parameter | What it shows |
|---|---|
| Tissue architecture | Presence of abnormal cells or cancer |
| Cellular atypia | Degree of abnormal cell growth |
| Inflammation | Signs of infection or prostatitis |
| Benign prostatic hyperplasia (BPH) changes | Evidence of non‑cancerous enlargement |
Why it is done
- To confirm or rule out prostate cancer when PSA is elevated or a digital rectal exam (DRE) is abnormal.
- To evaluate symptoms such as urinary difficulty, pain, or bleeding that may suggest prostate disease.
- To monitor known prostate cancer or evaluate response to treatment.
Early detection
This test can help identify signs of prostate cancer, but it does not diagnose on its own. A doctor interprets the result together with symptoms, history, and other tests.
What abnormal results may mean
- High cellular atypia or cancer cells – may indicate prostate cancer.
- Inflammation – could suggest prostatitis or other infections.
- BPH changes – indicate benign enlargement. Many factors (medication, infection, recent ejaculation) can influence findings.
Who might be advised to get this test
- Men aged 50+ with elevated PSA or abnormal DRE.
- Men with persistent urinary symptoms or hematuria.
- Patients with a family history of prostate cancer.
Summary
A prostate biopsy with ultrasound guidance is a targeted tissue sampling technique used mainly to detect or rule out prostate cancer and to evaluate other prostate conditions.
Used for detecting / monitoring
Before your test
Preparation
- Fasting: No fasting required for a prostate biopsy. If you are also having a blood test that requires fasting, follow the fasting instructions for that test.
- Medication: Continue your regular medications unless your doctor advises otherwise. If you are on anticoagulants, inform the clinician.
- Hydration: Drink plenty of water the day before to keep the bladder full for better ultrasound imaging.
- Avoid alcohol and strenuous exercise 24 hrs before the procedure.
Procedure overview
- You will lie on an exam table and a small probe will be inserted into the rectum to visualize the prostate.
- Under ultrasound guidance, a needle is inserted through the rectal wall into the prostate to take 12–14 core samples.
- The whole process takes about 15–30 minutes.
- You may feel pressure or mild discomfort during needle insertion; local anesthesia is used.
After the procedure
- You can resume normal activities after a few hours, but avoid heavy lifting for 24 hrs.
- Watch for rectal bleeding or pain; contact your doctor if symptoms worsen.
